Can I manage other family members on my account?

Yes, but the patient is required to sign a patient portal authorization form.
Access to any family member's account will be put on hold or declined without the authorization form completed. This form requires signatures from both the family member and the proxy requesting access. You can sign this form by visiting your physician’s office. You can also print this form and return back to your physician’s office, or scan and email the form to Brittany Bush, myCRMCHealth Coordinator at bnbush@crmchealth.org.
